In the early 1990s when I sold shoes in San Francisco the JTB tour people would talk about how この外人日本語うまい and I would (politely, cuz I wanted the sale) ask who was from the 外 in our particular pairing. In most cases in Japanese the word is used to mean "non-Japanese."
